New to Branch Family Medicine & Wellness?
We’re so glad you’re here. Before your first visit, here’s what to know:
- Appointments can be booked directly online through our patient portal.
- Please arrive 15 minutes early for your first visit to complete intake paperwork, or complete your forms online in advance through the patient portal.
- Bring a valid photo ID, your insurance card, and a list of any current medications, supplements, or vitamins.
- If you have records from a previous provider, bringing them (or having them sent in advance) helps us give you the best care from day one.
Lab Services
When lab work is ordered as part of your care, we’ll provide clear instructions on where to go and what to expect. We currently work with the following laboratory partners:

Lab & Diagnostic Turnaround Times
Please allow up to 5 business days for lab work and diagnostic imaging to be read, sent to your provider, and for results to be shared with you via the patient portal or a phone call. We prioritize appointment care first, so please allow this window before following up — any critical results will be communicated to you right away.
Telehealth Visits
Virtual appointments are available for established and new patients located in Illinois and Indiana. Telehealth is a great option for:

Chronic disease check-ins
Medication refills and management follow-ups
Acute illness consultations (non-emergency)
Medical weight management follow-ups
Wellness and lifestyle consultations

To join your telehealth visit: You will receive a secure link via email or text prior to your appointment. Simply click the link at the time of your visit from your phone, tablet, or computer.
No special software required.
Insurance & Billing
We currently take BCBS and Cigna, but are adding new insurance providers each month. Cash pay and concierge payment options are also available. For questions about your coverage or a bill, please contact our office directly.
Referrals & Specialist Care
When specialist care is needed, we’ll coordinate referrals for you. We maintain relationships with South Suburban Hospital and University of Chicago Medicine (courtesy privileges pending) and work with a broad network of specialists throughout the South Suburbs and greater Chicagoland area.
Our team will communicate with your specialist to ensure continuity of care and follow up with you after your specialist visit.
Prescription Refills
For routine prescription refills, please submit a request through the patient portal at least 1 week before your medication runs out. For controlled substance refills or medications requiring monitoring, an in-office or telehealth visit may be required.
Please note: medication refill requests are not processed over the weekend.
After-Hours & Emergencies
For medical emergencies, please call 911 or go to your nearest emergency room immediately.
For urgent but non-emergency concerns after business hours, please use the after-hours messaging option through the patient portal and we will respond as quickly as possible.
After-hours messaging is available through the patient portal (Tebra). If you are experiencing a medical emergency, please do not wait for a response — go directly to your nearest emergency room or call 911. Please note: medication refill requests are not processed over the weekend.
