Whether your child is heading into a new school year, your athlete is preparing for their season, or you need a DOT physical for employment certification, Branch Family Medicine & Wellness provides prompt, comprehensive physical examinations to get you where you need to go.

Types of Physicals We Offer
School Physicals
Required for school enrollment; health assessments for students of all school age. Includes a thorough physical exam, immunization review, vision screening, and documentation as required by Illinois state requirements. Please note: ongoing primary care management is available for patients age 13 and older; school physicals themselves are available for school-age children of any age.
Sports Physicals
Pre-participation physical evaluations (PPE) for athletes preparing for organized sports. We assess cardiovascular health, musculoskeletal fitness, and overall readiness to ensure your safety on the field or court.
DOT Physical Examinations
Federally required physical examinations for commercial drivers seeking or renewing their DOT Medical Examiner’s Certificate (MEC). DOT Examiner certification in progress — confirm availability at time of booking.
Employment Physicals
Required health assessments for employment or occupational purposes, tailored to employer specifications.
Veteran Exams
Veteran examinations (often called C&P exams) are a critical step in securing your disability benefits and healthcare. These medical evaluations document conditions related to your military service.
What to Bring to Your Physical
- Valid photo ID
- Insurance card (if using insurance)
- Immunization records (for school physicals)
- List of current medications
- Eyeglasses or contacts if applicable
- Any required forms from your school, athletic organization, or employer
